2024 Patient Experience Survey highlights

Each year, our PCN puts out a Patient Experience Survey to gather feedback from our patients. The optional survey gathers data from patients who had at least one appointment with our psychologists, nurses, or social workers. 

 

 The results of our 2024 survey results are in: 

  • 94 per cent of respondents who had an appointment with one of our Registered Nurses with the Senior Services program were very satisfied or satisfied with the care they received.
  • 88 per cent of respondents who had an appointment with one of our Primary Care Registered Nurse were very satisfied or satisfied with the care they received.
  • Patients appreciated that our psychologists and social workers offered appointments outside of normal business hours, allowing them to access services they otherwise would not be able to.
  • Social work patients said their providers helped connect them with key services to support their well-being.
  • Almost all respondents who had an appointment with one of our psychologists said they felt better prepared to deal with daily challenges because of their visit.
  • Nursing patients said their providers explained things in a way that was easy to understand, involved them in their care decisions, and listened carefully to their needs. 

We are happy to see that our programs and services are connecting our community with the resources and support they need to support their well-being.

Senior services available to you

 By 2030, more than one in five Albertans will be over the age of 65,  according to provincial estimates. 

 

At our PCN, we recognize the vital role older adults play in our communities — and we’re committed to supporting their health, independence, and well-being through our Senior Services Program. 

 

Our Senior Services team provides coordinated, team-based care designed specifically for older adults and those who are physiologically aged with complex needs. 

 

They help patients and caregivers navigate a wide range of health considerations, including: 

  • Medication review and education
  • Cognitive and functional assessments 
  • Fall prevention and nutrition risk screening 
  • Navigation of community resources and health services 
  • Patient advocacy and caregiver support 

Whether you’re 65 or older, or caring for someone who is, and managing chronic conditions, feeling isolated, or planning ahead, our goal is to ensure you feel supported, informed, and connected. 

 

Ask your family doctor if they are part of our PCN and whether a referral to the Senior Services team is right for you.

Stay on top of your health with the Men’s Health Checklist

When was your last checkup? Are you up to date on screenings and vaccinations? Managing stress, sleep, and lifestyle habits? 

 

If you’re not sure where to start, the Men’s Health Checklist from the Canadian Men’s Health Foundation is a simple, free tool that can help you stay on track with your physical and mental health. 

 

 This downloadable checklist is designed to help men of all ages:  

  • Track key health exams and vaccinations
  • Know when to schedule routine screenings
  • Monitor lifestyle factors like stress, alcohol use, sleep, and more
  • Take small steps toward lasting improvements

It’s an easy way to keep your health in check and prevent small issues from becoming bigger problems. 

 

You can download the checklist right away and even share it with your doctor at your next appointment. It’s also a helpful tool if you’re supporting a man in your life like a partner, son, parent, or friend and want to encourage them to take action. 

 

At our PCN, we’re encouraging men to take charge of their health by focusing on small steps that can make a big impact. From checking in with your doctor to learning about common health risks, it all starts with one conversation.
