More than just a checklist, a habit tracker is a structured tool that allows you to record progress on specific wellness habits throughout the week. As we reflect on 2025, we are grateful for a year filled with progress, collaboration, and meaningful impact across our community. At our PCN, our teams continued to work together to support patient needs, expand access to care, and strengthen the programs and services available to families across Calgary.
A few highlights that shaped our 2025:
Expanded our Physiotherapy Program: In the fall, the doctors in our membership could begin referring their patients with chronic low back pain (the leading cause of years lived with disability worldwide) to our program for assistance.
Provided more appointment options: The 2025 Patient Experience Survey showed that evening and weekend Mental Health Program appointments were well received, demonstrating the value of offering care at times that better fit patients’ schedules.
Strengthened partnerships: We continued to collaborate with local organizations to connect patients with resources that support physical, emotional, and social well-being.
Shared stories from our teams: We spotlighted the work of our nurses, psychologists, dietitians, social workers, corporate staff, and doctors in our membership. Offering a closer look at how our teams support patients every day.
Received national recognition: Our organization received national recognition for workplace safety and organizational excellence. These acknowledgments reflect the dedication, professionalism, and compassion of our teams.
Promoted connection: Through Community Health Connect and our social platforms, we reached thousands of people with practical health information and trusted local resources.

Alzheimer’s Awareness Month: Supporting seniors through community-based care
By 2030, researchers project that nearly one million people in Canada will be living with dementia.
January is Alzheimer’s Awareness Month, a time to recognize the impact of dementia and highlight the importance of early support for older adults and their families.
Alzheimer’s disease affects memory, thinking, and behaviour. While a diagnosis can feel overwhelming, early recognition can help families access care, plan, and better support their loved ones.
Why early recognition matters
Early signs can be subtle and may include:
Memory loss that affects daily activities
Difficulty completing familiar tasks
Confusion about time or place
Noticing changes early allows families to connect with resources sooner and understand what support may be needed.
How our PCN’s Senior Services helps
Our specialized Senior Services team provides integrated, team-based care to older adults aged 65 and up with medical issues. The comprehensive team provides access to physicians, geriatric psychiatry, pharmacy, and nurses certified in seniors' health.
Senior Services can:
Complete comprehensive assessments, including cognitive, functional, environmental, medication, fall, and nutrition screening
Advocate for patients and caregivers
Recognize and support individuals who are isolated or at risk
Help patients navigate healthcare and community support systems
This collaborative approach ensures older adults receive individualized care that reflects their health goals and priorities.
Supporting wellness at home
Many older adults want to stay at home as long as possible. Our team helps with:
Daily routine planning
Medication and health teaching
Fall-prevention strategies
Tips to support cognitive function
Caregivers can also feel overwhelmed. Access to clear information, reassurance, and community resources can make a meaningful difference.
